Following is the checklist for an Adventure traveller that can be used in your next adventure trip. This is a generic list and everyone can choose to pick of leave items based on their needs. The basic idea of packing during an adventure trip is to pack as less as possible but still accommodate everything that is essential in that back pack.

Necessary Documents:
Visa
Passport (Without this document you can't even imagine of entering a different country so make sure it's up to date and valid and stays so till the end of your adventure).
Carry an international Driver's License; it'll help you being independent in a foreign country.

Always carry some local currency with you as well
Book bus, airline, railway tickets early. In case you can make arrangements for your travel at that place even better.
Book your hotel bookings/ accommodation well in advance
Health Insurance
Book for any sporting event, festival or concert which you wish to attend well in advance.
Trip Cancellation Insurance
Most importantly, carry a couple of Photocopies of all the necessary documentation and keep them separately from the original ones.
Back Pack:
A fanny pack or hip pack would be a good option.
A travel pack which is sufficiently strong and durable.
In case you don't have it, duffel would be fine.

Sunscreen Lotion in accordance with the environment of the destination.
Water Bottle or some hydration mechanism like Ultimate Directions or Camelback.
Nylon Cord at least 50 meter in length.

Before you begin your journey, it's recommended to consult your physician once and make a list of all the medicines you'll require on your journey.
